Briggs Form 882 provides information
necessary for proper patient care when transferring to another facility.
Includes information on advance directives and vitals. Sending and receiving
facilities keep one copy of this 2-part form. If an extra copy is needed, order
Briggs Form 882T, a 3-part set.
Both forms printed two sides different. The back contains information regarding
the patient s ability to perform ADLs and his/her psychosocial disposition.
After completing the front side, pull out feather-edge carbon, turn form over,
reinsert carbon face down, and complete second side.
8 1/2" x 11" (detached), white
paper, black ink. Form 882 packaged in 125s and Form 882T packaged in 250s.
