EZ Pathways Skilled Nursing Discipline Booklet Wound

EZ PathwaysTM is comprised of a family of 30 disease-state specific clinical pathways.  One booklet covers one episode of care and is designed to:
Define the specific patient goals.
Identify the health status of a patient at any point in time.
Incorporate two, three or more co-morbidity interactions on a single page.
Identify standards of practice at each visit.
Numerically quantify areas of improvement, stabilization or decline.
Provide verification of required teaching and skill building.
Provide measured evidence of progress toward goals.
Acts as a nurse s note and outcome measures all on one page.
